- Once we receive your donation, we’ll e-mail you an emblem use authorization statement and form DOR-1716 which is an application for a Missouri Personalized & Special License Plate.
Fill out the application form (DOR-1716); submit it with the Emblem Use Authorization and the $15 special plate fee to:
